This project aims to design, develop and test a web-based toolkit that will enhance the capacity of Canadians living with chronic diseases and their caregivers, to find services and resources that could meet their needs.
People living with chronic diseases and their caregivers often feel lost and confused when attempting to navigate our health care system. Not knowing where to turn to for help, they often end up relying on busy hospital emergency rooms for non-urgent problems, with unsatisfactory results. Social workers, nurses and other professionals trained to guide people to services that could meet their needs are insufficient to meet demand. Meanwhile, relevant community-based information and referral services are underutilized. Hence, this projects aims to develop the Health eConcierge, a web-based toolkit that will enhance the capacity of Canadians living with chronic diseases and their caregivers, to find services and resources that could meet their needs.
