Centralizing a patient’s needs on one single platform to empower self-care management

Medly is a platform of apps that enables self-care to empower patients, addressing the following chronic complex diseases: chronic kidney diseases (CKD), chronic obstructive pulmonary diseases (COPD), congestive heart failure (CHF), and diabetes. Available on Android, the app features a questionnaire-styled toolkit that walks users through symptoms they are experiencing, centralizes lab results, tracks symptoms records and medications, and provides tips and training to better self-care management. Medly motivates patients to actively participate in their own self-care.


Test trial with patients with hypertension showed:

  • 150pg/mL improvement in Brain Natriuretic Peptide (BNP), indicating better heart function
  • 4% improvement of left ventricular ejection fraction
  • 7 point increase in self-care

What’s Next?

Medly CKD and COPD is currently being piloted within UHN sites. In the near future, Medly COPD, CHF, and diabetes will be deployed through the North York General Hospital.
